You need to start getting the feeling of piano keys again.

1. Your fingers need to be warmed up by playing technical skills (scales, chords, variations)
2. Start with easy piece that you can memorize then progress gradually. Master it!
3. Repeat 1&2 every day

I have been playing piano for 18 years now and had ever taken a break before as well :) visit my website for reference.
